U.S. Department of Health and Human Services – State Implementation Grants for Integrated Community Systems for Health Care Needs (CSHCN) 

The purpose of this initiative is to provide support to statewide implementation of the Health Resources and Services Administration (HRSA) component of the Presidents New Freedom Initiative to create inclusive community based systems of service for children and youth with special healthcare needs. With this initiative, HRSA’s Maternal and Child Health Bureau has the lead for developing and implementing a strategy to achieve appropriate community-based services.

Applicants must be or work closely with the State Title V Program for Children with Special Health Care Needs, have a complete and current statewide needs assessment that addresses the States status in each of the six systems components, work with partners to develop and implement a statewide plan to achieve a community based system of services using evidence based and best practices recommended by the Maternal and Child Health Bureau, integrate the plan into the Title V Block Grant, define a comprehensive evaluation plan using national, state and community data and demonstrate capacity to gather and report on systems outcomes.

The entire fund of $1,800,000 will be distributed between six selected recipients. The estimated project period is June 1, 2009 to May 31, 2010.
