Florida Department of Health, Office of Minority Health Funding – Reducing Racial and Ethnic Health Disparities: Closing the Gap Grant Program

The Reducing Racial and Ethnic Health Disparities: Closing the Gap Grant Program strives to eliminate racial and ethnic health disparities in Florida by providing grants to local counties and organizations that work to increase community-based health promotion and disease prevention initiatives. Specifically, the grant program seeks to eliminate health disparities regarding adult and child immunization, cancer, diabetes, cardiovascular disease, HIV/AIDS, maternal and infant mortality and oral health.

Approximately $5 million will be awarded in FY 2009-2010 for projects within Florida counties and Front Porch Florida Communities. An anticipated 40-50 awards will be granted. Eligible applicants are individuals or organization active in community-focused, collaborative efforts, which work to bring together agencies, community groups, academic institutions or other groups to address health or social concerns. Public and for-profit organizations applying for the grant program should have a record of service.
