The Substance Abuse and Mental Health Services Administration, Center for Mental Health Services - Primary and Behavioral Health Care Integration Grants 

The purpose of the Primary and Behavioral Health Care Integration grant program is to improve the physical health status of people with mental illnesses by supporting communities to coordinate and integrate primary care services into publicly funded community mental health and other community-based behavioral settings. Particular attention will be afforded to proposals that focus on individuals with serious mental illnesses who are served in the public mental health system.

An expected 11 grants will be awarded from a total of $5,500,000, although budgets cannot exceed $500,000 in total costs. Eligible applicants include publicly funded community mental health and other community-based behavioral health agencies.
