Mobilizing Action Toward Community Action (MATCH): Community Grants Program

The goal of MATCH is to strengthen the ability of communities to work together to translate the County Health Rankings, which provide data on population health and list the factors that need to be improved, into action. Additionally, this initiative seeks to engage funders and partners from a range of sectors in local efforts to improve population health. Proposals should address one of the following: education, income and employment, family and social connectedness, or community safety.

Fourteen awards of up to $200,000 will be awarded and grantees will have 24 months to implement their proposal. Eligible applicants include 501(c)(3) nonprofit organizations, state or local agencies, and tribal groups.

Deadline: April 27, 2011
