Substance Abuse and Mental Health Services Administration, Center for Mental Health Services – Healthy Transitions Initiative (State Governments and Tribal Organizations)

The Substance Abuse and Mental Health Services Administration, Center for Mental health Services, is accepting applications for the Cooperative Agreements for State/Community Partnerships to Integrate Services and Supports for Youth and Young Adults 16-25 with Serious Mental Health Conditions, and Their Families (Healthy Transitions Initiative). The Healthy Transitions Initiative will create developmentally-appropriate and effective youth-guided local systems of care to improve outcomes for youth and young adults with serious mental health conditions in education, employment, housing, mental health and co-occurring disorders, and decrease contacts with the juvenile system.

An expected 7 awards will be granted to selected applicants for up to $480,000 per year. Eligible applicants include State governments and federally recognized American Indian/Alaska Native Tribes and Tribal organizations.
