New York State Department of Health -- Healthy Heart Program (NY)
The New York State Department of Health’s Healthy Heart Program is seeking applications for projects that will create and promote new or expanded, community-level, sustainable opportunities that make it easier for residents to be physically active or chose a healthy diet. Selected interventions should focus on policy and environmental changes and be evidence-based. Priority will be given to projects that serve a predominantly low-income urban or rural area.
A total of $950,000 in funding is available for 12 awards. Eligible applicants are public sector and other non-profit organizations in New York State.
