Affordable Care Act Family-to-Family Health Information Centers (Select States)

The goal of this funding opportunity is to develop Centers that provide information to parents of children with disabilities and special health needs. These Centers should assist families in making informed decisions about health care in order to identify and promote good treatment decisions, cost-effectiveness, and improved health outcomes for such children. In addition, Centers should develop a model for collaboration between families of children with special health care needs (CSHCN) and health professions; and conduct outreach to families and schools with CSHCN.
Twelve awards of up to $95,700 will be available. Eligible applicants include public and private entities, faith-based organizations and community organizations in the following states only: AK, AL, AR, IA, ID, KY, NM, OH, SC, WA, WV and WY.
