About the CQI tool authors


Linda Juszczak, MS, MPH, DNS, is former president of the National Assembly on School-Based Health Care. Her professional experience includes work as a clinical nurse practitioner and a manager for community-based and school-based programs for adolescents. She also served as deputy director of the School Health Policy Initiative at Montefiore Medical Center. Dr. Juszczak has authored numerous journal articles and book chapters. During the past year she has collaborated with colleagues Drs. Christopher Reif and Doris Pastore, to develop a quality improvement tool for school-based health centers. Dr. Juszczak received her undergraduate degree from Skidmore College, a masters in nursing at the University of Colorado, a Pediatric Nurse Associate certificate and a masters in public health from the University of Minnesota, and a doctorate in nursing from Yale.

Doris Pastore, MD, is Associate Clinical Professor of Pediatrics at Mount Sinai Hospital in New York and director of its 6-site school-based health center program. After completing a pediatric residency at Albert Einstein College of Medicine, she trained as an adolescent medicine fellow at North Shore University Hospital/Cornell University Medical Center where she practiced at the Far Rockaway School-Based Health Center. She received the Society for Adolescent Medicine Young Investigator of the Year award in 1991, and has authored papers and book chapters on school-based health care as well as a variety of topics within adolescent health. During the past year, she has collaborated with Drs. Reif and Juszczak in developing a continuous quality improvement tool for school-based health centers.

Christopher J. Reif, MD, MPH, MA is director of community medicine, Department of Family & Community Medicine at Health Partners Regions Hospital in St. Paul, Minnesota. He has provided medical care to the seven school-based health centers in St. Paul for more than a decade, and serves as medical director for the St. Paul Homeless Health Care program. Over the past ten years, he has also served as medical director for the Ramsey County Department of Corrections, the Consortium of Community Clinics in Minneapolis and St. Paul and the Helping Hand Community Clinic. Dr. Reif graduated from St. John's University in Minnesota, received a masters degree from the University of Colorado, and and a medical degree and masters in public health from the University of Minnesota. During the past year, Dr. Reif has joined Drs. Juszczak and Pastore in developing a quality improvement tool for school-based health centers.
